Authorization and Consent FormPermission to record Online Tutoring Sessions for training purposes Information received in this consent form will be treated as confidential and is being gathered for the purposes of improving the quality of tutoring provided to your child by Vancouver Urban Ministries. Any information collected here serves to authorize Vancouver Urban Ministries for the school year 2020-2021.Tutor Training Vancouver Urban Ministries is currently providing free, one-on-one online tutoring to your child. In order to maintain and improve the quality of tutoring we provide, we are seeking your permission to record occasional tutoring sessions between your child and their tutor. We will advise the Parent/Guardian when the tutoring sessions are being recorded for training purposes. The recording will have no impact on your child’s experience of tutoring.Vancouver Urban Ministries Commitment with regard to recorded tutoring sessions: 1. Recorded Tutoring sessions will be treated with complete confidentiality and will be viewed only by the tutor and the Tutoring Director 2. Recorded tutoring sessions will NOT be used to assess your child’s performance but only for tutor training purposes. 3. Recorded tutoring sessions will only be shared between the tutor and the Tutoring Director and only to enable training/advice to be given to the tutor. 4. Recorded training sessions will NOT be shared online (either on social media or on our website or on any other online capacity) or be used for any other purpose other than for tutor training. 5. After the tutoring sessions have been reviewed between the tutoring director and the tutor, they will be completely deleted and will not be retained or stored by Vancouver Urban Ministries. 6. Recorded tutoring sessions will not be used in any Vancouver Urban Ministries advertising or promotions. 7.
